For this rotation, my site evaluations were completed via Zoom with my site evaluator and two other students who rotated at different sites.
During the mid site evaluation, each of us presented one patient case/H&P. For this presentation, I presented a case of cellulitis I had seen at the urgent care where I am rotating. This was a case I had chosen, as I saw the patient both at the initial visit as well as the follow-up appointment. For this case, my site evaluator discussed the value of demarcating the affected area at the initial visit to better assess for improvement or worsening/persistence of the infection at the follow-up visit. He also mentioned how taking a picture of the affected area at the initial encounter to compare at follow-up would similarly be beneficial.
For our final site evaluation, each of us presented another patient case/H&P followed by a journal article related to our case. I presented a systematic review and meta-analysis that evaluated the efficacy of corticosteroids as an adjunct in the treatment of tonsillitis and acute pharyngitis in addition to analgesics, such as acetaminophen and NSAIDs. Our site evaluator reaffirmed the article’s findings that an IM injection of a corticosteroid provides benefit, as he similarly manages patients with such diagnoses by treating them with Dexamethasone and Ketorolac in the ED. Management of these cases was helpful to learn, as they are commonly seen in both the ambulatory and ED setting.
